To put it simply, physical examination is a very core implementation method in the preventive health care system. The two are nested rather than parallel. The difference is mainly reflected in timeliness and coverage. The correlation is mutually supportive and indispensable.
To use an inappropriate analogy, a physical examination is like a high-definition snapshot you take of your body regularly, which can only reflect your health status during the days of the examination. The core goal is to uncover abnormal indicators and early lesions that do not show symptoms for the time being, such as pulmonary nodules, breast nodules, and marginal increases in blood lipids and blood sugar. These are difficult to detect based on daily feelings, and basically require physical examination to accurately capture them. Preventive care is more like a whole process of health care that runs through your entire life. It ranges from proactively wearing a mask during the flu season, taking the elderly and children for vaccinations on time, to dietary adjustments and exercise planning after high blood pressure is detected, and even rehabilitation follow-up and recurrence prevention guidance for patients with cerebral infarction after discharge. These are all covered by preventive care. It is continuous and is not done once and for all.
There are actually two extreme voices in the industry now. One is that "I spend thousands every year to get the most comprehensive physical examination, which is equivalent to doing good health prevention." The other is that "physical examinations are IQ taxes, and taking good care of your health is the real preventive care." Both of these opinions are actually a bit wrong. Take a patient I met in my previous community health management post. Uncle Zhang, 58, has a 30-year history of smoking and undergoes routine physical examinations at work every year. Last year, he was suddenly diagnosed with advanced lung cancer. When his family asked about reports from previous years, they discovered that he only took ordinary chest X-rays every year, and early ground-glass nodules could not be detected at all. Moreover, reports from previous years have indicated that the lung texture has thickened. Our family doctor reminded him to do a low-dose lung CT during the follow-up. He also felt that it was unnecessary. He always felt that there was nothing wrong with him because he did not have a cough or chest pain. He neither adjusted the physical examination items nor did targeted smoking control and other health interventions, which meant that the value of the physical examination was not fully realized.
The opposite is also true. It is easy to miss early risk signals just by paying attention to health and judging your health status by your own feelings. For example, I have met many Internet practitioners in their twenties who usually pay attention to fitness and drink less milk tea. They feel that they are already doing preventive health care. However, many people already have problems with high blood lipids and high uric acid. They don't feel it at all. They can only find out through physical examinations and adjust their lifestyle in time to nip the problem in the bud.
In fact, to put it bluntly, the relationship between the two is not either/or at all. Physical examination can help you accurately find the targets of health risks, and preventive care can solve the problems of these targets one by one. At the same time, it can help you establish healthier living habits and reduce risks from the root cause. Only by working together can you truly take your health into your own hands.
