    The trocar used in children's circumcision is a common instrument-assisted surgical method. It mainly blocks the blood supply of the foreskin through ligation so that it can fall off naturally. This operation needs to be completed by a professional doctor under sterile conditions. Commonly used instruments include disposable plastic cerclage or commercial rings.

    Before surgery, the doctor will assess the degree of phimosis and provide local anesthesia. After disinfection, a special dilator will be used to gently separate the adhesions between the foreskin and glans. The inner ring is placed in the coronal sulcus of the glans penis, and the outer ring is placed on the outside of the foreskin. The inner and outer rings are tightly fitted through elastic ligation or instrument locking. The ring will compress the blood circulation of the distal foreskin, causing ischemic necrosis. The ligated foreskin tissue gradually shrinks and dries in 5-7 days after the operation, and the ring will fall off along with the necrotic foreskin in about 10-14 days. The ligation strength needs to be precisely controlled during the operation to avoid being too tight and damaging the glans or being too loose and causing bleeding. The ligation ring will retain the urinary channel after surgery. This method has less bleeding and neat incisions, but may cause edema, pain or premature detachment of the ring.

    It is necessary to keep the perineum clean and dry after surgery and avoid strenuous exercise that may cause the cuff to shift. Disinfect the wound with iodophor 2-3 times a day, and dry it promptly after urination to prevent urine soaking. Wear loose cotton underwear to reduce friction. If bleeding persists, the ring deviates, or does not fall off for more than 3 weeks, follow-up is required. Swimming and straddle sports are prohibited within 1 month after surgery. It is advisable to eat a light diet and drink plenty of water to prevent urinary tract infection.
